You will need:

1. A computer and internet connection.
2. An email address.
3. A receipt with a survey invitation printed on the front.


1. Visit the website on your computer.
2. Enter your 16 digit store code that is printed on the front of your store receipt.
3. Select which survey you wish to participate in. Your receipt will have the correct survey address printed on the front.
4. Answer all the survey questions honestly and candidly.
5. Provide your name and email address.
6. The validation code will be sent to your email address where you can then write it down on the back of your old store receipt.
7. Hand to the cashier on your next visit to PetSmart for discount or free items.

My husband and I went into the Laplace store on last Sunday morning and as we were waiting to check out because the cashier had several customers in line, we asked if someone else was able to check us out and to no avail. My husband knocked on a door where a large white lady came out of the office looking strangely and seemed to be on her cell phone. My husband told her that there is a line, can she help. Everytime we come into this store, this particular woman is in that office. My husband knocks on it all the time. She is never out there to help these other associates.

  2. Some of the print to light for reading comfortably. On my other laptop computer, I can not do the survey, something to do with cookies. This laptop I can do the surveys, do not know what the difference is. The time given of 48 hrs is too do the survey is not adequate, easy to forget the time frame given. The till receipts are hard to read for the elderly persons.

  3. Clear and easy to fill out. No problems at all.

  4. I could not get this web page to do anything after putting in my pin number from my receipt. This page needs to be working if you ask people to do a survey for your company and they take their time to try and complete it.

    Print too hard to read. Scratchy. Form fills itself in all by itself.

    spent 20 minutes trying to fill out survey – not friendly- not happy-make it simple
    ONE – Suggestion: give customer a coupon when they check out to use the next
    time they shop. You have just lost me as a customer!!!!!!!

    cant read the web site print is like a bad typewrtier that is why no one is doing your survey plus too much work for $3 I had to type this twice so no way baddddddddddddd
