Staples Win $5000 Gift Card Survey

You will need:

1. A computer and internet access.
2. A Staples store receipt.
3. Be over 18 years of age and a legal resident of the US.


1. Visit to begin the official Staples survey.
2. Enter the 16 digit survey code printed on your store receipt into the space provided. Start the survey.
3. Enter the date and time of your visit as well as the store number and amount you spent.
4. The survey questions will ask you to rate your shopping experience on a scale of Highly Satisfied to Highly Dissatisfied. Answer each question honestly and use the extra space provided to elaborate on your experience if necessary.
5. Answer some general demographic questions about your age, race and gender.
6. If you wish to enter the monthly $5000 Staples Gift Card Sweepstakes you will need to submit your personal contact details. This is to make sure you can be found if you are the winner.

  2. I have tried for 30 minutes to locate the Staples survey to no avail. My visit to the Toms River NJ store was not enjoyable either.

  3. Allie at the Kingston, NY store was so helpful with printing out a legal-sized contract I emailed Staples and then helping me to determine the best way to mail it to my agent. Ten stars!

    Had excellent experience at the Lindero Canyon oad Store, West Lake Village, CA. Left my reading glasses home and your sales associate, Anthony, came to my rescue reading the HP catalogue to find the right ink cartridge. He also went the extra mile in assisting me on looking at other items.

  6. I purchased pens and pencils at the Berlin, VT store. The woman at the checkout was very kind and had a great smile.

    I have not been able to access the survey this evening through any of the links on this site or several others.

  7. I had a not so good experience with the associates and manager’s at the Titusville store. But, the associates and management at the Melbourne, FL store #1055 redeemed your company for me. The 1055 associates were extremely friendly and helpful over the phone and in the store. I also received top notch assistance with my reward coupon not showing up in my reward acct. Then manager actually contacted the rewards dept. and verified my reward and was able to add to my purchase right then and there! Fantastic customer service, that is how ALL your stores should operate. Also, I had an great experience with the customer service dept. over the phone.

    We were looking for a new faster laptop that would work with my embroidery machine and Zachary and Andrew from the Berkeley Plaza Staples in Goldsboro NC were awesome very knowledgeable and helpful. After checking other places even visiting other Staples stores we found that they couldn’t compare to you guys, they couldn’t even order the computer for us so we came back to our local Staples and you guys ordered it for us and You guys are the BEST !!!!

    In short words,i have a great experience in stapples because im not have to much in u.s.a im come from Puerto Rico to get a Job and i need some stuff for prepare a couple of resumes i dont know anything of here,and for a surprise for me,im look on a little bench and found a stapples shopper and i say(thank you LORD¡) perhaps i found there what i need…and guess what,yes i found it¡….thank you stapples and stapples staff.

  11. I had a great experience at Staples today, and wanted to complete the survey, but the address on the receipt does not take me directly to a site that I can use. It says “enter the 16 digit survey code.” My receipt has a 19 digit code in the location where the website seems to be pointing. I could not expand the picture on the first website I found in order to assure I was using the correct code, and the resolution of the picture on the second website is so poor, it is hard to tell. Sorry, I tried….. The sales lady was awesome at the Hershey, PA store. She knew what I was looking for (notebook spine index labels), realized her store no longer carried it, took me to the kiosk and searched for something on line, and when she did not find it, came up with an alternate solution. She was great!

    I went into my local Staples (4320 Broadway NY,NY) with a ton of question about my custom print order. Likely, Brenda was there to guide, assist and ease my mind and my order. Her knowledge and pleasant demeanor was a customer’s dream. Thank you Brenda and wonderful job on my order…….just perfect!!!!!!

    I purchased a desk chair for my son yesterday. Candace from the Bishop’s corner store helped us with the purchase. She not only got the chair out of inventory for us, she brought it to the front cashiers area and rung it up for us herself right away. We received excellent customer service from Candace and will be back for future needs.

    We had the need to purchase a new computer and we had the great opportunity to be assisted by Harry in the Glendale store. He was unbelievable in the thorough manner he explained all the options available to us and our needs. The way he answered our questions allowed us to make a decision that is exactly right for us. Thank you Harry!
    We had a large printing need at the same time so left awaiting the completed call. When we returned Harry had finished his shift and departed but we had another associate, Jose Cohen, assist us. He explained all the options available to us in a friendly manner so we could make the correct decision for us.
    The store manager was also friendly and his attitude and knowledge is reflected in those who he is responsible for.
